Client Profile & Navigation

Profile Header

The client profile header displays:

  • Full name
  • Status (Active / Inactive / Incomplete)
  • Creation method
  • Start of care date
  • Service types (HC / HH / Both)

Dynamic Left Navigation

The left nav is shown or hidden based on the client's service type. Admins only see sections relevant to the services the client is enrolled in.

OASIS Assessment is a clinical outcome tracking requirement under Medicare/Medicaid Conditions of Participation. It applies to Home Health and Both clients only — never to Home Care only clients.

Admin Capabilities

From the client profile, admins can:

  • Edit client demographics
  • Upload and delete documents
  • Re-send digital forms for signature
  • Manage service plans and visit records (HC and/or HH)
  • Review compliance records and health information
  • Monitor document completion status
  • Maintain audit records

Section Descriptions

Documents

Admission documents, consent forms, medical records, uploaded files, and system-generated forms. Admins can upload files, delete documents, re-send unsigned forms, and monitor completion status.

Service Plan

Authorized services, visit frequencies, disciplines, and care objectives. When a client is enrolled in Both, separate HC and HH service plans are maintained.

Service Visit

Visit type, assigned staff, visit dates, and status/documentation. Supports operational planning, billing, and clinical tracking. Separate HC and HH visit records when enrolled in Both.

OASIS Assessment

Home Health and Both clients only. Clinical outcome measurement and tracking required for Medicare/Medicaid regulatory reporting.

Supervisory Reports

Records of supervisory visits. For HH clients: RN supervisory visits are required at least every 14 days; in-home direct observation at least every 60 days.

Satisfaction Surveys

Client satisfaction data collected across the care relationship.

Case Communications

Messaging and notes between the care team and the client record.

Incident Reports

Logged incidents associated with the client's care.

Incomplete Documents

Documents assigned to the client that have not yet been completed or signed.


Delete vs Deactivate

Two distinct actions are available when removing a client from active caseload:

Default to Deactivate. Delete should be a secondary, explicitly destructive option, always gated behind a confirmation dialog.
